Rhinoplasty is sought by both men and women for a variety of reasons; in essence, this tailor-made procedure is used by surgeons to alter the size, shape and/or trajectory of your nose. 

This is achieved by manipulating the structure of the nose via small incisions, be that the hard bone at the bridge, the softer cartilage at the lower dorsum nasi (length of the nose), the soft tissue and skin around the nostrils—or a combination of these—to produce the desired aesthetic result. 

Common cosmetic reasons why patients seek rhinoplasty treatment are that they perceive their nose to be:

• Too large (more common in the West)

• Too small (more common in Asian countries)

• Aquiline (prominent bridge)

Each step is outlined in brief below:

Step 1. Anaesthesia

First, it should be noted that a nose job can be performed under local anaesthesia with IV sedation, or general anaesthesia. Usually taking between one and two hours, it is most commonly performed on an outpatient basis at an accredited surgical facility, meaning that patients leave the same day after a few hours of monitoring. 

In some cases, especially for those with pre-existing medical conditions that may complicate the administration of general anaesthesia, patients may require an overnight stay in hospital for further monitoring. Ultimately, your doctor will be able to recommend the best choice for you.

Step 2. Incision(s)

Once the anesthesia has taken effect, surgery can begin. Nose job procedures use either:

• A small incision made across the columella, the strip of tissue separating the nostrils (open procedure); or

• Incisions that are hidden inside the nose (closed procedure)

The skin covering the cartilage and nasal bone can then be carefully raised via these small incisions, allowing the surgeon to reshape the structure of the nose as described below in step 3. 

Step 3. Structural Reshaping

In cases where a patient is seeking a nose reduction, bone and/or cartilage may be removed. 

For the opposite effect, cartilage grafts can be inserted into the nose via a small incision to add length or bolster its structure. Grafts from the septum may be used for this purpose, as well as cartilage harvested from the ear.

Step 4. Correction of a Deviated Septum (Optional)

In cases where a patient has a deviated septum, this can be straightened via the same incisions, with breathing improved by reducing any projections inside the nose.

Step 5. Closing the Incision(s)

Once the desired shape and angle of the nose has been achieved through surgical restructuring, the opened flaps of skin and tissue are redraped and the incisions are sutured closed to complete the procedure. The nose may also be supported with splints and then bandaged to provide protection.

• Changes in skin sensation or discolouration

• Undesirable scarring

• Anaesthesia risks

• Swelling

• Unsatisfactory appearance

• Infection

• Slow wound healing or scarring

• The bruising and swelling may vary depending on the exact nature of your procedure, but it could be a couple of weeks before it has settled down enough for you to return to work.

• After surgery, your nose is likely to be bandaged, but you may also have splints and your nose may be packed with gauze (which should remain in place for 7–10 days.

• Most patients return home after 3–5 days. 

• You will want to be doubly careful for a few weeks that you don’t bang your nose 

• You will be advised not to do any exercise for three weeks, and even then to take it gently. 

• Over the next few weeks, your nose will begin to feel stronger and after six weeks you should be able to resume normal activities and vigorous exercise.
