Each step is outlined in brief below:
Step 1. Anaesthesia
First, it should be noted that a nose job can be performed under local anaesthesia with IV sedation, or general anaesthesia. Usually taking between one and two hours, it is most commonly performed on an outpatient basis at an accredited surgical facility, meaning that patients leave the same day after a few hours of monitoring.
In some cases, especially for those with pre-existing medical conditions that may complicate the administration of general anaesthesia, patients may require an overnight stay in hospital for further monitoring. Ultimately, your doctor will be able to recommend the best choice for you.
Step 2. Incision(s)
Once the anesthesia has taken effect, surgery can begin. Nose job procedures use either:
A small incision made across the columella, the strip of tissue separating the nostrils (open procedure); or
Incisions that are hidden inside the nose (closed procedure)
The skin covering the cartilage and nasal bone can then be carefully raised via these small incisions, allowing the surgeon to reshape the structure of the nose as described below in step 3.
Step 3. Structural Reshaping
In cases where a patient is seeking a nose reduction, bone and/or cartilage may be removed.
For the opposite effect, cartilage grafts can be inserted into the nose via a small incision to add length or bolster its structure. Grafts from the septum may be used for this purpose, as well as cartilage harvested from the ear.
Step 4. Correction of a Deviated Septum (Optional)
In cases where a patient has a deviated septum, this can be straightened via the same incisions, with breathing improved by reducing any projections inside the nose.
Step 5. Closing the Incision(s)
Once the desired shape and angle of the nose has been achieved through surgical restructuring, the opened flaps of skin and tissue are redraped and the incisions are sutured closed to complete the procedure. The nose may also be supported with splints and then bandaged to provide protection.
